To all established patients prior to September 27th, 2023:
Please be advised that as of September 27th 2023, we are transitioning to new electronic medical record and administration systems software. Due to this, we have a new patient portal. Your existing login information is not valid for this portal and you will need to create a new account with our new portal.
If you have not received an email from “welcome@practicemailer.com” with instructions to create your new account, please call us at 615-656-4055.
